For Linux users (only a few of us, I know), I would also add Kstars to the planetarium programs and software list. It's a very nice free and scriptable planetarium program, with lots of features.
So here's the latest with my update:

I went into Costco the other day and ran across a book called “The Illustrated Atlas of the Universe” and since it was only $20 I picked it up. I do not yet have a star atlas (Waiting for this project to get going!) so I really do not know if it will qualify for this listing . It is a big book (11x13) with about 300 pages; stars begin on about page 200. The front has summary info about the moon, planets and space flight with lots of nice pictures. The star maps are organized by constellation and go down to magnitude -6.5. It struck me that it would qualify but I will find someone local with one from the list and see how it compares. If it looks appropriate I will write up a piece for this list once it gets ready. Even if it does not make it to the list, I found it very enjoyable to read and well worth the expense.

In addition to what you have listed, I'd make note of how far south the coverage extends. If it's pole to pole, that's surely something our southern hemisphere comrades would appreciate knowing. (I'm assuming this is what you mean by 'all sky' but I'm not certain this would be clear to the average newbie.)
Also, are the appendices broken down at all into subheadings? If so, listing what sort of information (glossaries, etc.) might be of interest.
What you have suggests a good standard format:

Note that I've added a line for listing the weight of the tome. I don't know about the rest of you, but weight (every bit as much as dimensions) determines how easy it is to use something at the eyepiece, or whether or not I leave it on the table.

Yeah, come on out and start picking my draft apart.
They're out there Tom, but not everyone gets online every day and it might take a while for interested parties to join in.
Btw, there's something which probably should have been included in my draft copy.. appearance of the charts. I'll edit my draft after posting this, but it's a good thing I originally forgot this stuff. It pertains to something which needs to be addressed at this early stage, but first, the new info:
Chart background: medium blue Milky Way: light blue Stars and deep sky objects: yellow Constellation related lines: white Ecliptic and co-ordinate lines: black Lettering: black Print size: averages 1/8" to just a bit under 1/8"
Now, the issue it brings up.. objectivity.
Our purpose in writing these fact-sheets is to present information as dryly as possible. This will not only provide the reader with non-biased information, but it will also encourage members to ask for opinions and present questions in the linked discussions.
I found it very helpful to stick to the facts by using a disciplined list-type structure. An essay style would have made it too easy for me to slip up and mention that the 1/8" black print on the blue background might be difficult for some readers to see.

The additions are a good idea. They'd serve as a fallback if we can't get permission to reproduce a chart, and as something of an interpretive aide if we do.

Carol's summary lacks a chart picture (if we can cross that copyright hurdle) to make it really useful.
Also, I looked at the summary to see whether I could figure out what I was getting with the book after reading the write up, and I think I need something more than just a "card catalogue" write up to let me know about the book:
Info such as: limiting magnitude of the charts, number of DSOs in the entire book, how much info is printed about each object (if any), whether "binning" or "infinitely variable" scale is used for star images and objects, whether the objects have their correct sky position angles on the charts, whether there is overlap on the object designations, what catalogs are used in DSO selection, scale of the map in degrees or minutes per inch of paper, changes from previous printings (if known), whether chart overlays are available/provided.
I'm sure other things could be reported that I didn't mention above. A dry, objective write up should include all of the above and more. I trust others will chime in here...........?

My personal preference is for reviews that mix both fact sheet and subjective info. In the Lunie Bin a while back, Mardi did a killer review of the S&T/Rukl moon map. That fit the bill for me for a review. I see the fact sheet format and I immediately go blank, a la Homer Simpson.
(Of course, I spend most of my life going blank anyway, so that may not mean anything).

Don, thank you for asking these questions. You brought up points which I'd have never thought to include, but they'll definitely help people get a better picture of what the charts look like.
And as a bonus, you've given me an idea... for a 'standardized fact-list'. Suppose we make a formal, numbered list of points we'd like to see covered in the fact-sheets, and then use that standard form for all publication write-ups? Naturally, some points wouldn't apply to all publications, but supplying the same info for all publications would certainly insure that contributors wouldn't overlook important points like I did. Numbering the points would make it very easy for readers to compare one publication with another, too. Sound reasonable?

I agree that the essay form is best for reviews, but the proposed thread was never intended to contain reviews.. just factual information.
Of course if the contributor wants to, they can always post their personal comments about the publication in the linked discussion thread or (better yet) write up a CN review and submit it for publication in the 'Article Discussions' forum.
