I use a PDA for Pocket Stars for observing. So I use Pocket Excel on the PDA to keep my notes and I make the notes as I observe. That keeps all my astronomy stuff together and I don't have to transcribe anything later.

Attached is an Excel spreadsheet that I've used. Feel free to change modify and massacre it.
