Does anyone have any suggestions on how to install a post on the flat top of a surveyor's tripod so that the azimuth adjusting screws of a GEM mount can be used in polar aligning?

I've replaced a number of "shaky" telescope tripods with surveyor tripods. I use simple blocks of wood to make the adapter plates. It should be relativity easy to make a pillar out of wood.

This is an easy fix. Simply drill a hole in the top plate and put a hex head bolt in. Align the flats of the bolt head to the az adjusting screws. You may have to place a few flat washers under the bolt head to have the proper alignment.

If you want it to look "professional", you can buy a stainless steel threaded stud from McMaster-Carr:
http://www.mcmaster.com/#standard-threaded-rods/=kds4wr
But StarSuff's idea will work just as well.

Where should this hole be drilled? I've heard in line with one of the tripod legs. True? And how far from the outside edge of the tripod top plate?

Yes, in line with one of the tripod legs.
How far back from the edge? As far as it needs to be for your mount. There is no "standard" distance for all mounts.

You could always buy a CG5 adapter plate from Universal Astronomics, that adapts a CG5 to the top plate of a surveyors tripod. The post for the az-adjustment is built-in and works well.
