QHY 268C Flats Issue

I use Sequence Generator Pro and am unable to get a flat frame with a 268C. Darks, bias and lights are all fine. Each frame looks like the below image

I see this split screen occasionally when using frame and focus but never in the lights. I've tried sub second frames to 7s frames and get the same result. I've read of this happening occasionally with both the QHY and ZWO versions. Any suggestions?

Processed mono, without CFA selected shows a different thing altogether

Update: I put the 294M on the scope and flats look perfectly fine. Very frustrating

Hmmm, I have this QHY268c camera and never had or seen before an issue like this with flats, I use 4 second flats with mine and matching dark flats

I’ve found a handful of identical cases going back several years, both QHY and ZWO. I’d love to know the cause
